CURRENT PATIENTS

Our preferred method for the video visits is Zoom. Apple FaceTime is an acceptable alternative for established patients. Michigan laws requires video connections for virtual sessions. Phone or audio-only connections are not an option. If you do not have access to a video method we will need to cancel your appointment. If you are unfamiliar with the video method you have chosen on your completed follow-up form, we request that you practice ahead of time with a family member or friend.

IMPORTANT: new FORM DEADLINES

We ask that your form be submitted at least one business day prior to your appointment.

If we don’t receive your form:

• By 4:30pm the day prior to your appointment: we may begin offering the appointment time to patients on our wait list

• 2 hours before your appointment: the appointment will be reassigned to another patient. You will then be placed on the wait list with no guarantee of next availability.
